Proof of Identity and Proof of Address documents are generally needed to apply for new AADHAAR enrolment. In case these documents are not available, Certificate of Identity, having photo, issued by Gazetted Officer / Tehsildar on letterhead is also accepted as valid Proof of Identity. Certificate of Address, having photo, issued by MP or MLA/Gazetted Officer / Tehsildar on letterhead or by Village Panchayat head or its equivalent authority (for rural areas) is accepted as valid Proof of Address. In that case, along with AADHAAR enrolment application form, the applicant will also have to submit "Certificate For AADHAAR Enrolment / Update" form, duly certified by the respective authority, if he / she is using any of these documents:
- Proof of Identity:
- Certificate of Identity having photo issued by Gazetted Officer or Tehsildar on UIDAI standard certificate format for enrolment / update
- Certificate from Superintendent / Warden / Matron / Head of Institution of recognized shelter homes or orphanages etc. on UIDAI standard certificate format for enrolment / update
- Certificate of Identity having photo issued by MP or MLA or MLC or Municipal Councillor on UIDAI standard certificate format for enrolment / update
- Certificate of Identity having photo issued by Village Panchayat Head or Mukhiya or its equivalent authority (for rural areas) on UIDAI standard certificate format for enrolment / update
- Certificate of Identity containing name and photo issued by Recognized Educational Institution signed by Head of Institute on UIDAI standard certificate format for enrolment / update.
- Certificate of identity containing Name, DOB and Photograph issued by Employees' Provident Fund Organisation (EPFO) on UIDAI standard certificate format for enrolment / update
- Certificate of Address having photo issued by MP or MLA or MLC or Gazetted Officer or Tehsildar on UIDAI standard certificate format for enrolment / update
- Certificate of Address issued by Village Panchayat head or its equivalent authority (for rural areas) on UIDAI standard certificate format for enrolment / update

- Certificate of Date of Birth issued by Group A Gazetted Officer on UIDAI standard certificate format for enrolment / update
- A certificate (on UIDAI standard certificate format for enrolment / update) or ID Card having photo and Date of Birth (DOB) duly signed and issued by a Government authority
- Certificate of Identity containing Name, DOB and Photo issued by Recognized Educational Institution signed by Head of Institute on UIDAI standard certificate format for enrolment / update
- Certificate of identity containing Name, DOB and Photograph issued by Employees' Provident Fund Organisation (EPFO) on UIDAI standard certificate format for enrolment / update
- Certificate of Identity having photo issued by MP or MLA or MLC or Municipal Councillor or Gazetted Officer on UIDAI standard certificate format for enrolment / update
- Certificate of Identity having photo and relationship with HoF issued by Village Panchayat Head or Mukhiya or its equivalent authority (for rural areas)

The same certificate is applicable for both, new AADHAAR application enrolment and for AADHAAR correction / update. Print the certificate for AADHAAR enrolment / update form on a plain paper. The certificate for AADHAAR enrolment / update form consists of two parts: Resident's Details, which the applicant has to fill and Certifier's Details, which the respective certifier will fill and authorize.
Instructions on how to fill certificate for AADHAAR enrolment / update form:
- Fill all the required details in the certificate form; do not leave any mandatory information empty.
- Do not overwrite in the certificate form; in case of mistakes discard and use a new certificate form.
- Fill the certificate form carefully in capital letters only, using only standard fonts (avoid stylized writing) for easy readability and to minimize chances of any human mistakes.
- Fill the certificate for AADHAAR enrolment / update certificate form using black or blue ball point pen only, do not use ink pen or pencil to fill the certificate form.
- Date values must be filled properly in DD/MM/YYYY format.
- Put a tick mark only in the boxes where options are to be selected as the answer, leave the other option(s) blank.
- Leave the boxes empty where the requested detail is non relevant; do not write NA or N/A or NOT APPLICABLE.
- Write clearly in the centre of the box without touching the box boundaries.
- While filling the field, leave one box blank between each complete word.
Instructions for filling Resident's Details in certificate for AADHAAR enrolment / update form:
- Date of Issue: Date on which the certificate is issued should not be older than 3 months from the date of AADHAAR enrolment / update request. Application with expired certificate or empty Date of Issue value on the certificate will be rejected.
- Resident Category: Tick mark the appropriate box depending upon whether the applicant is a resident of India or an NRI.
- Enrolment Type: Tick mark the appropriate box depending upon whether the application is for a new AADHAAR enrolment or for AADHAAR update request.
- Aadhaar Number: For new AADHAAR enrolment, leave this blank; for AADHAAR update request mention the applicant's AADHAAR number.
- Full Name: Mention the applicant's name. The name must match exactly as is mentioned in the AADHAAR enrolment / correction / update form.
- C/o: If required, mention the Care of (C/o) in the address field, else leave this blank.
- Address Fields: Mention the address details in appropriate fields; all fields are mandatory except Landmark and Post Office.
- Date of Birth: Mention the applicant's date of birth.
- Signature: Applicant shall put his / her signature inside the given box. Illiterate applicant can provide thumb / finger impression.
- Photograph: Applicant shall paste recent color photograph of size 3.5cm X 4.5 cm inside the given box. The photograph will need to be cross signed and cross stamped by the certifier.
Instructions for filling Certifier's Details in certificate for AADHAAR enrolment / update form (to be filled by the certifier only):
- Name of Certifier: Mention the name of the certifier.
- Designation: Mention the designation and office name of the certifier.
- Office Address: Mention the complete address of the certifier, along with the department name.
- Contact Number: Mention the contact phone number details of the certifier.
- Certifier Type: Tick mark the appropriate box for the certifier type.
- Checklist for Certifier: Certifier shall verify and tick mark all the boxes to confirm the certificate form is filled properly.
- Sign & Stamp of the certifier: Certifier shall put his / her signature and stamp inside the given box.
